• Player’s name and Jersey number
• Parent’s name and contact information
• Association or AAA program currently involved with.
• Copy of Practice\Game schedule or Team’s schedule Website link
• Level (A, B1, etc or 1999, 2000, etc)

Team Selection Assessment:

1. Receive an invite directly from an MLH Board member or the Coaching Staff
2. Attend a tryout Assessment and be invited as a member of a team
3. Contact a MLH coach directly to have your child assessed during a regular season practice or Game
